Back to School News from the Nurse

Medication forms for students are now available on the Skyline Schools website. Click on the OTHER tab and drop down to NURSE’S CORNER. There you will find non-prescription, prescription, and self carry administration forms. Please print them off and fill them out prior to the first day of school.

Prescription forms will need to be signed by your child’s physician, and all forms will need to be signed by parents prior to administration of the medication. If you want your child to be able to carry their prescription medication with them (i.e. inhalers) your physician will need to sign a self carry form. If your doctor has their own form regarding prescription medications and self carry medications then those can be faxed to the school and will be accepted as orders for medications.

Please remember that if you wish for your child to have tylenol, ibuprofen, benadryl, cough drops, or any other non-prescription medications kept in the nurse’s office, you will need to bring the form and a new UNOPENED bottle of medication to school. This will be labeled with your child’s name for their use only. Siblings may use the same bottle and may be placed on the same non-prescription form.

Please feel free to bring medications to enrollment or to open house prior to the first day of school. I am looking forward to seeing all of your children again. Have a great rest of the summer!
